Providence Centralia Hospital is located at 914 South Scheuber Road in Centralia.

URGENT UPDATE: Providence Swedish is actively seeking former patients and their family members to join the Patient Family Advisory Council (PFAC). The council aims to enhance patient care and services at Providence Medical Group clinics, as well as at Providence St. Peter Hospital and Providence Centralia Hospital. Applications are now open for individuals eager to share their insights and experiences.

Since its establishment in 2011, PFAC members have played a crucial role in shaping policies and practices directly impacting patient care. “Providence Swedish in South Puget Sound is committed to partnering with patients, families, and caregivers throughout the healthcare experience,” stated Chief Executive Darin Goss. “The Patient Family Advisory Council provides an invaluable perspective. Our mission is all about serving patients and families. With PFAC members serving on committees, the eyes of the patient are always with us.”

The council meets on the first Friday of each month from 12:00 PM to 2:00 PM. Currently, all meetings are held in person and are available via video conferencing for those unable to attend in person. Providence Swedish encourages individuals from diverse backgrounds to apply, ensuring that the council reflects a wide range of experiences and perspectives.

The application process is straightforward: interested candidates must submit an application, participate in an interview, and complete the volunteer recruitment process. To apply, fill out the application at shorturl.at/qtVDM and email it to Trish Warnick at [email protected]. For any inquiries, you can also reach Warnick by calling 360-791-3486.
